This puzzle adventure horror game follows Doug Dunaheiw, a convicted murderer who discovers that the door to his cell in an abandoned hospital is unlocked. He takes his stuffed bear, Teddy, and sets out to find a way out. An ancient evil has taken hold of the hospital, blending reality with dreams, while monsters and traps lurk in its corridors.
The player explores the hospital, solves puzzles and uses their wits to deal with its dangers. The viewpoint shifts between an overhead perspective and first-person view depending on the situation. The story unfolds through journals, notes and conversations with other survivors, while Doug has an inventory and a journal.
